Hospital Diversion Program
Right Care. Right Time. Right Place.
Beacon’s Hospital Diversion Program provides safe, community‑based alternatives to inpatient hospitalization for individuals experiencing mental health or substance use crises. By partnering directly with local hospitals and Community Mental Health Centers, we help people access the right level of care at the right time—often outside of an emergency department.
Through real‑time referrals from hospital partners, our team engages individuals who may not need inpatient care but still require immediate support. We work to reduce unnecessary hospitalizations by connecting people to services that promote stabilization and long‑term recovery.
Key program functions include:
- Receiving direct referrals from hospitals
- Providing alternatives to emergency department admission
- Supporting both mental health and substance use needs
Connecting People to the Right Support
When appropriate, individuals are redirected to services that offer safety, structure, and immediate support. These options help people stabilize in the community while avoiding avoidable hospital stays.
These supports may include:
- Crisis stabilization programs
- Outpatient therapy and treatment
- Peer support and recovery services
A Personalized, Trauma-Informed Approach
Our team meets individuals where they are—physically and emotionally. We focus on trust, dignity, and person‑centered care. Outreach and case management ensure each person is connected to the services that best support their recovery.
Common areas of support include:
- Therapy and counseling
- Medication management
- Housing and benefits resources
- Community service navigation
The Hospital Diversion Program strengthens access to timely, appropriate care. It reduces pressure on emergency departments, helps people stabilize more quickly, and increases connection to ongoing supports. Ultimately, the program promotes safety, stability, and lasting wellness.
