Requesting Records

Our office can provide same day copies of records of Diagnosis, Current Medications and/or full history of past Medications, Labs, Treatment Plans, and a List of Appointments and Services.

For any other records, including Provider Progress Notes and Assessments, a request form must be completed to request access from the Provider or Chief Clinical Director. This process can take 7-10 business days and there will be a $6.50 flat fee.

To Revoke a previously signed authorization, please submit a statement in writing to the attention of our Privacy Officer. Your statement should include your name, date of birth, and the name of the individual or agency on the authorization you are wishing to revoke.
